矢状面バランスを失った成人脊柱変形症例の手術で目指すべきアライメント—代償のないアライメントを目指して

　矢状面バランスを失った成人脊柱変形症例に対する矯正固定術時の矯正目標について述べる．脊柱変形では局所の変形に対してさまざまな代償性のアライメント変化が生じる．高齢者の広範囲固定例では，これらの代償のないアライメントに矯正すべきである．それには骨盤後傾を改善する必要がある．そこでわれわれは骨盤後傾を改善するために必要な腰椎前弯角を算出するフォーミュラを考案した．十分な腰椎前弯に矯正すると，胸椎や下肢の代償はなくなり，代償のない良好なアライメントが獲得できる．
