頚椎Pedicle Screw

概要

　筆者が1990年に開始した頚椎椎弓根スクリュー固定は，神経血管系統へのリスクの懸念から，当初大きな批判にさらされた．しかし，その強力な固定性は頚椎の再建手術に大きな変化をもたらした．また，同法の登場は，頚椎の椎弓根自体および椎骨動脈などに関連する形態学的研究ならびに頚椎内固定の生体力学研究の進歩も促した．2000年代に入って，頚椎椎弓根スクリューの正確な刺入を企図し，種々のaiming deviceの考案やnavigation systemの援用もなされてきた．さらに同法の持つリスクを忌避し，C2およびC7椎弓スクリューや椎間関節スクリューも考案され，頚椎後方のinstrumentation手術の様相は大きく変化した．前世紀末までは，頚椎の変形矯正手術は脊椎外科領域に残された課題の1つで，積極的な取り組みは稀であったが，強力な固定性により頚椎配列異常の矯正が可能な頚椎椎弓根スクリューの登場は，頚椎変形の矯正手術の進歩を後押ししたといえる．
