背景

　1994年当時も，臼蓋形成不全に対する代表的な手術的方法は寛骨臼移動術(TOA)¹¹⁾と寛骨臼回転骨切り術(RAO)¹⁰⁾であった．TOAとRAOは弯曲ノミを用い骨盤外側から股関節の周囲を同じように球状に骨切りする方法で，骨切り後の密着性と骨癒合に極めて優れている．私も大転子の骨切りに工夫を加えたTOAを30数例に行っていた⁷⁾．この方法の欠点と思われたのは，外側アプローチによる骨盤外側の外転筋群の剝離が避けられないことで，移動寛骨臼や中臀筋の血行が一時的にせよ不良になるのではないかと危惧した．欧米ではSmith-Petersenアプローチで進入するperiacetabular osteotomy(PAO)²⁾が代表的な骨盤骨切り術として普及し始めていた．PAOでは中殿筋を部分的にしか剝離しないため寛骨臼の血行は保たれるが，すべて平面での骨切りであるため移動寛骨臼は台形に近い形状となる．このため，PAOでは，回転させた寛骨臼と骨盤の母床との間に間げきが避けられず，また，寛骨臼自体が前方へ移動する可能性も考えられた．
