骨折の疫学と転倒予防

　骨折の発生率や予後の疫学についてのエビデンスを，4つの骨脆弱性骨折について述べる．脊椎圧迫骨折は転倒をせずに発生する割合が高いのに対して，それ以外の非椎体骨折は転倒を契機に発生しており，受傷機転や転倒方向の違いによってさまざまな形態の骨折が発生していると考えられる．非椎体骨折の多数は骨密度が正常ないし骨減少領域で発生していることが明らかになっており，その予防には骨粗鬆症の治療に加えて転倒予防が重要である．高齢者の転倒予防介入を有用とする報告は数多くあるが，骨折予防については骨粗鬆症治療に比べて効果が高く確立した方法はいまだなく，今後の研究・発展が期待される．
