外固定期間中に尺骨頭が掌側脱臼したGaleazzi骨折の1例

　外固定期間中に尺骨頭が掌側脱臼した稀なGaleazzi骨折の1例を経験した．症例は35歳，男性で，スケートボード中に転倒し，転位がわずかな橈骨骨幹部骨折を受傷した．前医で保存的治療を行っていたが，受傷後2週で骨折部の背屈転位が増悪し，尺骨頭が掌側脱臼した．受傷後3カ月で当科に紹介された．橈骨骨幹部骨折を整復内固定することにより遠位橈尺関節は整復され，さらに三角線維軟骨複合体直視下縫合術を追加することで，不安定性が消失した．橈骨骨幹部骨折の治療では，遠位橈尺関節の損傷を念頭におき，骨折部を整復し強固に内固定することが重要である．
