腰椎椎間関節内側切除術の長期成績—後根神経節の除圧の重要性

　背景：腰椎椎間板ヘルニア・腰部脊柱管狭窄症の手術について，神経の除圧術と脊椎固定術の技術的進歩があるが，その治療効果についてはいまだ議論があり，未解決である．

　対象と方法：後根神経節の除圧を含む椎間関節内側切除術を施行し，10〜40年(平均16.9年)の直接検診による追跡調査のできた671例について，臨床成績を分析した．

　結果：Excellentは79.6％，満足できるGood＋Excellentは85.7％である．Fairは1.6％，Poorは1.8％，再手術は10.9％であった．JOAスコアは術後28点前後を推移し，経過しても低下しない．脊椎不安定性の有無と臨床成績は統計的に関連性が認められなかった．

　まとめ：本手術は簡単な手技で，神経因子とくに後根神経節の除圧を正確に施行すれば，長期に極めて良好な成績が得られ，推奨できる．
