難治性テニス肘の直視下手術

　上腕骨外側上顆炎に対するさまざまな手術手技が紹介されているが，直視下手術・経皮的手術・鏡視下手術などのいずれの術後成績にも差を認めないとする結果が多く，現時点ではどの手術法が優れているのか一定の見解はない．現在筆者が行っている最小侵襲での直視下手術は，局所解剖を熟知していれば確実な病巣部の掻爬に加えて，輪状靱帯の切離や関節内の処置も同時に安全かつ極めて短時間に行え，さらに重篤な合併症や再発の発生もなく術後成績は良好であり，外来治療に抵抗する難治例に対して広く推奨できる手術法と考えられる．
