腰仙椎移行部椎間孔狭窄の病態—腰仙椎椎間孔の形態とL5神経根の走行に着目して

　背景：腰仙椎移行部椎間孔狭窄の病態はいまだ十分解明されていない．

　対象と方法：本症に手術加療を行った85例中，症状を認めた計100椎間孔を検討した．

　結果：前後狭窄は46椎間孔，頭尾狭窄は77椎間孔で認め，Type 1(前後狭窄のみ)は22椎間孔，Type 2(頭尾狭窄のみ)は53椎間孔，Type 3(前後・頭尾狭窄合併)は24椎間孔，Type 4(far-out syndrome)は1椎間孔であった．前後狭窄は主に椎間孔内側で認め，神経根は斜走したままで，頭尾狭窄は主に椎間孔外側で認め，神経根は横走化する傾向にあった．

　まとめ：L5/S1椎間孔の内外側で，それぞれ特有のL5神経根障害が起こる可能性が考えられた．
