仰臥位前方進入法により骨接合術を行った大腿骨骨頭骨折の1例

　股関節前方進入法(DAA)は人工股関節全置換術(THA)での筋温存型進入法である．今回，われわれは外傷性股関節後方脱臼に伴う大腿骨骨頭骨折に対してDAAで進入し，観血整復術を施行した1例を経験したので報告する．

　症例は32歳，男性で，スキーで転倒し受傷した．救急搬送先の病院で左大腿骨骨頭骨折(Pipkin分類type Ⅱ)を伴う股関節後方脱臼の診断で，全身麻酔下に徒手整復が施行され，受傷翌日に当院へ転院となった．当院でDAAによりポリ-L-乳酸(PLLA)スクリューを用いて骨接合術を施行し，短期ではあるが良好な成績を得たので報告する．
