症例報告

脊髄くも膜下出血により急速に対麻痺を来した1例

　症例は79歳女性で，心房細動に対してワーファリン内服中であった．突然の背部痛と両下肢のしびれで発症し，MRIで第1〜5胸椎高位に脊柱管内血腫を認め，脊髄は高度に圧排されていた．麻痺は進行性で，発症16時間後には下肢筋力がMMT0〜1となり，緊急手術を行った．硬膜外腔には血腫を認めず，くも膜下腔に多量の血腫を認め，脊髄くも膜下出血と診断した．術後1年の時点でFrankel分類Cの麻痺が遺残していた．術後3カ月のMRIでくも膜囊胞を認めたが，神経症状の悪化や囊腫の増大傾向を認めず，現在，経過観察中である．
