肘関節リウマチに対する中間膜挿入関節形成術の経験

　メトトレキサート(MTX)の服薬でもコントロールできない疼痛を主訴とする肘の関節リウマチに対し，滑膜切除に併用して腸脛靱帯を中間膜とした関節形成術を行った．術後，疼痛の消失と関節可動域，日常生活動作の改善が得られ，術後3年でも満足な状態が維持された．この方法は，歴史的には古い方法であるが，関節リウマチの薬物治療が進歩し，関節の破壊性変化が減少し変形性関節症様の変化がみられるようになってきた今日では，人工肘関節置換術と比べ，bone stockが温存でき，術後の負荷荷重制限を厳密にする必要がないなど有効な方法と考えられる．
