片側の椎間板高減少（wedging）の進行による椎間孔狭窄—単純X線正面像でのスクリーニング

　目的：単純X線正面像で片側の椎間板高が減少(wedging)している例では，椎間孔は狭くなる．椎間板のwedging進行度が椎間孔狭窄のスクリーニングに有用かを検討した．

　対象と方法：腰椎手術を行った205例を対象とした．単純X線正面像で各椎間板のwedgingを計測し，椎間孔狭窄の有無を調べた．

　結果：椎間孔狭窄は，L5/Sで15例，L4/5で8例，L3/4より上位で0例に認められた．L5/Sに4°以上のwedgingがある25例では5例に椎間孔狭窄を認め，4°未満の例に比べ高率に椎間孔狭窄症状がみられた(p＜0.01)．

　まとめ：L5/Sに軽度のwedgingがあれば，椎間孔狭窄を念頭に置いて診断すべきであると考えられた．
