骨折を伴わない足関節開放脱臼の1例

　症例は21歳，女性で，バレーボールで受傷した骨折を伴わない左足関節開放脱臼である．外果周辺に約5cmの開放創を認め，緊急で洗浄・整復術を施行した．靱帯修復は行っていない．術後MRIで内果と距骨内側の軟骨損傷を疑い，受傷後12日目に足関節鏡検査を施行した．関節内の剝離した軟骨を摘出し露出した軟骨下骨にmarrow stimulation techniqueを行った．術後はギプス固定を6週間，軟性サポーターを6週間使用した．受傷後1年でストレスX線像で足関節不安定性を認めず，MRIでも距骨壊死の所見はない．JSSFスコアは100点で，バレーボールも受傷前と同様レベルで行っている．
