頚椎椎弓形成術を施行した頚椎症性脊髄症と頚椎後縦靱帯骨化症における10秒テスト/JOAスコア/JOACMEQの経時的変化，および後方除圧高位が頚部痛と頚椎機能に与える影響に関する前向き比較研究

背景：頚椎症性脊髄症(CSM)と頚椎後縦靱帯骨化症(OPLL)に伴う頚部脊髄症術前後の10秒テスト/JOAスコア/JOACMEQの経時的変化，後方除圧高位別の頚部痛と頚椎機能を前向きに比較検討すること．

対象と方法：頚椎椎弓形成術施行症例55例を対象とし，各スコアの術前から術後12カ月まで経時的変化を観察し，疾患別およびC7温存群とC7挙上群を比較検討した．

結果：各スコアに関して疾患群間に有意差を認めなかった．頚椎機能・頚部痛に関してOPLL群ではC7挙上の有無での有意差を認めなかった．

まとめ：OPLLはCSMと比較し手術治療成績・機能回復予後に有意差を認めなかった．OPLLにおいて術後骨化伸展が危惧される症例はC7挙上を推奨する．
