小児肘関節脱臼における合併骨折の検討

背景：小児肘関節脱臼は多くの場合で骨折を合併する．初診時に骨折の有無を適切に評価して治療方法を選択することが重要だが，後日に骨折が判明することも多い．当科で経験した小児肘関節脱臼の脱臼方向と合併骨折の関連や頻度を検討する．

対象：橈骨頭の単独脱臼を除く小児肘関節脱臼16例(受傷時年齢4〜15歳)を対象とした．

結果：16例中11例で骨折を合併していた．後外側脱臼に伴う内側上顆骨折が11例中4例と最多であった．

考察：脱臼方向から合併する骨折を予測することは，初診時の骨折の診断に有用と考える．
