成長期腰椎疲労骨折CT分類の信頼性—矢状断CT分類と横断CT分類の読影信頼性の比較

背景と目的：成長期腰椎疲労骨折はMRIで早期診断後，CTで骨癒合率を予測し治療方針を決定する．腹側から背側に向かって進行する骨折線の進行程度で評価する矢状断CT像分類を考案し，従来の横断像CT分類との検者内および検者間信頼性を比較検討した．

対象と方法：31例(38骨折)を横断CT5スライス像と矢状断CT3スライス像で再構築し，3名が各々を読影し，分類した．

結果：横断CTを参照しつつ，矢状断CTで進行程度を分類した結果の検者間信頼性Fleiss' kappa係数が0.582と最も高かった．

まとめ：急性期の成長期腰椎疲労骨折は，横断CTを参照しながら，矢状断CTで分類することが推奨される．
