誌上シンポジウム 高気圧酸素治療の現状と可能性

壊死性軟部組織感染症に対する高気圧酸素治療

　壊死性軟部組織感染症は，適切に治療されなければ死亡率30〜70％の致死性疾患である．高気圧酸素治療は，クロストリジウム性ガス壊疽に対しては，嫌気性菌の発育とα毒素の産生を抑制し，生命予後は大きく改善した．壊死性筋膜炎に対しては，白血球の抗菌作用の増強，嫌気性菌のみならず好気性菌の発育を抑制，低酸素状態の軟部組織の創治癒促進，抗菌薬の作用の増強などで，死亡率や切断率の減少につながることが報告されている．壊死性軟部組織感染症の治療において，全身集中管理のもと，早期の適切な外科的処置と抗菌薬の使用に高気圧酸素治療が併用されれば，死亡率や切断率の減少などの治療成績の向上が期待される．
