Modic signとは

　椎体終板変性は，MRIで日常的に観察される変化である．一般的には退行性変化として考えられている．図1で示すように，年齢とともに椎間板高が減少し，その変化が椎体終板に発生する現象である．高齢者の8割以上はこのような画像を呈している．変性腰椎の椎体終板変性はModicら¹⁾により報告され，一般的にはModic signと呼ばれている．椎体終板はMRIのT1強調画像で低輝度，T2強調画像で高輝度を呈するModic Type 1，T1強調画像，T2強調画像でともに高輝度を呈するType 2，さらにT1，T2強調画像で低輝度を呈するType 3に分類された^2,3)(図2)．最近のModic signのレビューによると，腰椎にその変性を認める割合は14％であり，変性の程度は年齢に比例し，10年間で6％の増加を認めることが報告されている．Modic signの分類ではType 2が多く，次にType 1であり，Type 3が最も少ない²⁾．多くの論文において椎間板の変性が腰痛の原因となりうることが報告されているが，椎間板の近傍に存在する椎体終板変性の病理と臨床的意義に関する論文は少ない．本稿では，現在までにわかっているModic signの臨床的意義に関して述べたい．
