人工股関節全置換術におけるMISの功罪

　低侵襲人工股関節全置換術(MIS-THA)と称される手術にもさまざまなものがある．単に皮膚切開長を短くしただけのものもあれば，可能な限り筋腱切離を行わないもの，可能な限り骨の切除量を減らして骨温存するものなどがあげられる．それぞれに対し，メリットとデメリットが存在するが，最も大きなデメリットは，ラーニングカーブの存在であり，それを克服しないと決して低侵襲とはいえない．さらにその低侵襲性が，長期的にみた場合にどれくらいのメリットがあるかは，それぞれの術式で異なる．しかしながら，手術侵襲を少なくするという目的の追究姿勢は重要である．工夫をし，より低侵襲な手術手技を確立していく必要がある．
