頚椎および腰椎椎間板ヘルニアに対する椎間板内ステロイド注入療法の効果

　椎間板内ステロイド注入療法(以下，SIDT)は椎間板ヘルニアの保存療法の1つである．今回われわれは，1997年以降に当科において頚椎もしくは腰椎椎間板ヘルニアの診断のもとSIDTを施行した98例の治療成績について検討した．疼痛の評価にはDenisのペインスケールを用いた．施行後24週以内に疼痛がペインスケールP3以下に改善し，最終観察時点まで悪化せずに手術を回避できたものを有効とした場合，有効率は37.8％であった．治療効果が早期から現れたこと，強い下肢痛を持つ症例や頚椎例にも高い有効率を示したことから，SIDTは椎間板ヘルニアの有用な保存療法の1つであると考えられた．
