四肢麻痺を呈した第4頚椎原発の非ホジキン悪性リンパ腫の1例

　四肢麻痺を呈した極めて稀な頚椎骨原発非ホジキン悪性リンパ腫を経験した．手術治療の有用性を報告する．症例は70歳の男性で，4カ月前から頚部から両肩甲部にかけての疼痛が出現し，前医を初診した．生検による第4頚椎骨原発非ホジキンリンパ腫の診断のもと，単独での後方固定術を施行した．しかし，術後早期に椎体への直接腫瘍浸潤による麻痺が出現し，当科初診時にはFrankel分類Bの四肢麻痺を呈していた．初診翌日に前方腫瘍摘出・固定術を緊急的に行い，術後2週の時点で化学療法を開始した．術後4週でFrankel分類Dまで麻痺は回復した．頚椎骨原発の悪性リンパ腫は渉猟し得た限り，4例の報告を数えるのみである．悪性リンパ腫の主たる治療法は化学療法であるが，手術療法も選択されうる．頚椎前方に病変がある場合では後方固定術単独では四肢麻痺発生の危険性があり，前方腫瘍摘出・再建術が1つの選択肢である．
