　手術部位感染予防で最も科学的根拠が強いのが予防抗菌薬である．本稿では，その使い方と問題点について骨・関節術後感染予防ガイドライン改訂版(日本整形外科学会)と術後感染予防抗菌薬適正使用のための実践ガイドライン草案(日本化学療法学会)を中心に概説する．また，術中術野汚染対策はわれわれ外科医として特に気になるところであるが，世界的にバイオクリーンルームの有用性に対し懐疑的な見方が強くなってきている．一方，術前除菌の有用性についてはさまざまなエビデンスが出てきた．予防を考えるうえで知っておくべきこれら最新のトレンドについて概説する．
