症例報告

再手術を要した後縦靱帯骨化症を伴った頚椎脱臼骨折の1例

文献概要

　再手術を要した後縦靱帯骨化症を伴った頚椎脱臼骨折の1例を経験したので報告する．症例は70歳，女性で，distractive flexion typeのC5/6脱臼骨折に後縦靱帯骨化症を合併していた．徒手整復で神経症状が改善傾向を認めたため，非除圧での前後方固定手術を行った．しかし，術後麻痺が進行し，MRIでC5/6の狭窄が増悪していたため，緊急後方除圧固定手術を行った．術後，症状は改善し，術後1年でFrankel分類D，骨癒合も得られた．既存の脊柱管狭窄がある強直性脊椎を伴った頚椎脱臼骨折の手術法は，後方除圧固定が望ましいと思われた．
