転移性脊椎腫瘍に対する根治的手術の適応

　腎細胞癌，甲状腺癌の脊椎転移は放射線抵抗性のため，脊椎転移巣の長期局所コントロールを得るためには根治的切除が理想的である．乳癌の脊椎転移では，放射線療法とビスホスホネート製剤か抗RANKL抗体の併用が第一選択となる．しかし，耐え難い痛みや進行する麻痺を認める場合には手術の適応となり，その際，脊椎の転移巣が単発転移で長期予後が見込める場合には根治的手術の適応となる．肺癌(腺癌)脊椎転移では，肺の原発巣がコントロールでき，肺内転移も含めた主要臓器やリンパ節への転移がなく，脊椎転移が限局している場合は，根治的切除も考慮すべきである．
