アキレス腱断裂の手術療法と保存療法における足関節底屈筋力回復時期の比較

目的：アキレス腱断裂に対する手術療法と保存療法との間で，最終的な筋力回復に差はないが，前者の筋力に後者の筋力が追いつく時期の詳細は不明である．本研究の目的は，この時期を明らかにすることである．

対象と方法：アキレス腱断裂に対する手術療法(14例)と保存療法(ギプス4週間固定8例，6週間固定7例)との間で，受傷後2，3，4カ月時の足関節底屈筋力の健患側比を比較して筋力回復の時期を調査した．

結果：2カ月時の足関節底屈筋力は手術群がギプス4週群と6週群に対して有意に高値であった．3カ月時にはギプス4週群と手術群との間で有意差が認められなくなり，4カ月では3群とも同様の値となった．

まとめ：手術療法が保存療法(ギプス4週群)に対して筋力回復で有利な時期は，受傷後2〜3カ月の間の1カ月間であった．
