人工股関節全置換術における後方アプローチ—アプローチ概論を含めて

　人工股関節全置換術(以下，THA)におけるアプローチは，単に股関節に到達するための“アプローチ”というよりも，むしろ術式の主要部分の1つである．どのようなアプローチを行うかによって人工股関節の安定性や機能が影響される．良好なTHA後成績を得るにはアプローチに対する理解が重要である．THAにはさまざまなオプションがあるが，アプローチ方法の選択により人工股関節の種類が制限される場合もある．この中で，後方アプローチは汎用性に優れており，さまざまなTHAでも対応可能である．また操作性も高く，これまで最も多く使われてきた．しかしながら，前方系のアプローチと比較すると術後後方安定性に劣り，閉創時の後方軟部組織修復が重要である．
