仰臥位前方進入法の基本手術手技

文献概要

　前方進入法は，後方進入法より股関節機能の回復が早いとされる唯一の最小侵襲手術であり，現在でも，世界各国で広がりつつある．手技的には，大腿神経に接近する臼蓋前壁へのレトラクター挿入に注意すること，股関節包内下方の血管処理を忘れないこと，カップやステムの過前捻設置に注意することなどが重要である．さらに，この進入法の導入時には，やせ形の女性で，軽度の臼蓋形成不全を有する，可動域のよい症例を選択することが大切である．特に，前方からの挿入には，扁平楔状型のステムが適している．
