胸椎OPLL手術の多施設研究—厚生労働省脊柱靱帯骨化症に関する調査研究27年間の概要

　胸椎OPLLは保存治療の効果が乏しく手術を要する疾患であるが，術前の高度脊髄症状，胸椎後弯，黄色靱帯骨化症併存などの特徴により手術成績は良好とはいえず，至適な手術方法も確立されていない．本稿では，1989年以降現在までほぼ27年間の胸椎OPLL手術に関し，旧厚生省，現厚生労働省の脊柱靱帯骨化症調査研究班によってなされてきた胸椎OPLL手術多施設研究を概説する．現在は胸椎OPLL手術多施設前向き研究が進行中であるが，これまでの班研究による知見の蓄積，instrumentationを含む手術手技の進歩，術中脊髄モニタリングの発展などとともに，胸椎OPLLの手術成績は向上している．一方，一過性の術後運動麻痺を含めた術後合併症率は依然として高く，手術方法の確立に向け多施設研究をさらに進めていく必要がある．
