整形外科医のためのH. pylori関連疾患とNSAIDs潰瘍の治療と予防

はじめに

　上部消化管疾患の治療を考えるうえでHelicobacter pylori(H. pylori)感染の有無は重要である．この細菌はミイラ(アイスマン；5,300年前に氷づけになったミイラ)の胃にも認められたように，人間と共存する微生物としては最古の部類に入るとされている．しかし，この細菌が分離・培養されたのは1982年であった．胃酸はpH1〜2の強酸であり，この酸による殺菌作用により胃内で細菌が住み続けることは不可能であると信じられたことも発見が遅れた原因であろう．H. pyloriの発見以降には急速に研究が進み，胃炎，胃潰瘍との密接な関係が明らかになり，1994年にはWHOがH. pylori感染は胃癌のdefinite carcinogenであると認定した．それゆえ，上部消化管疾患を考えるうえで常にH. pyloriを考慮しておく必要がある．本稿では整形外科医に知ってほしい胃疾患の治療と予防について述べる．
