脊椎手術後の深部静脈血栓症(DVT)スクリーニング検査としての可溶性フィブリン(SF)値測定の前向き研究

目的：可溶性フィブリン(SF)値が脊椎手術施行後の深部静脈血栓症(DVT)スクリーニング検査として有用なのかを検討した．

方法と対象：前向き研究により脊椎手術121例を対象に，術後1日目・4日目にSF値測定，術後3〜5日目に下肢静脈エコーを行った．

結果：SF値は術後1日目と比較して，術後4日目で平均値は低下したが，上昇する症例のほうが多かった．SF値の推移は，出血量や手術侵襲の影響を受けていた．2例に下肢エコーでDVTを認めたが，それを推定するSF値の特徴は認めなかった．

まとめ：SF値は出血量，手術侵襲の影響を大きく受けることがわかった．DVTスクリーニング検査として，今回の測定方法では有用とは言えなかった．
