腰下肢痛の最近の画像診断—新たな画像技術・評価法の有用性と限界

 近年,腰椎変性疾患に対する画像診断技術の進歩はめざましく,従来の方法では捉えられなかった腰痛・下肢痛の病態が把握できるようになってきた.それに伴って,得られた画像についての新たな解析・評価法についても多数の報告がなされている.本稿では,腰椎変性疾患に伴う腰痛・下肢痛の画像診断における最近の知見を紹介し,それらの臨床的な有効性と限界について検討する.今回は特に,1)馬尾・神経根の圧迫病変,2)椎体終板・椎間板変性,3)筋変性,4)脊椎アライメント異常,5)仙腸関節障害,6)脳の器質的・機能的変化の各病態の画像診断について述べる.
