—高齢者の腰痛—腰痛と骨粗鬆症性椎体骨折

　当教室で行った骨粗鬆症性椎体骨折に対する多施設研究の結果について概説する．1回目の研究から，約80％は予後良好であるが，骨癒合不全に至ると臨床上の問題が増え，予後不良例の予測においては，装具や入院などの治療的介入よりも，後壁損傷を認めること，T2強調画像の特徴的な所見，運動習慣のないことなどの要因が重要であることが判明した．2回目の研究からは，MRIでの骨折部の信号変化は長期にわたり残存すること，椎体骨折後の腰痛の推移には4つのパターンがあり，骨折部の安定化が疼痛緩和に寄与することが判明した．
