足関節外側靱帯損傷に対する鏡視下手術の最前線

はじめに

　近年の足関節鏡手技・器械の進歩に伴い，陳旧性足関節外側靱帯損傷の手術治療において，鏡視下手術に代表される低侵襲手術に関する報告が増えている^1,2)．鏡視下手術は遺残靱帯を用いる靱帯修復術，移植腱を用いる靱帯再建術の2つに大別され，遺残靱帯の状態が良好であれば靱帯修復術が，遺残靱帯の状態が悪ければ靱帯再建術が選択される．

　筆者らも遺残靱帯の状態によって，スーチャーアンカーを用いた鏡視下靱帯修復術³⁾，自家薄筋腱を用いた鏡視下靱帯再建術(A-AntiROLL)のいずれかを選択し行っている⁴⁾．

　本稿では筆者らが行っている陳旧性足関節外側靱帯損傷における鏡視下手術について述べる．
