椎間関節性腰痛に対する電気焼灼術

はじめに

　腰痛に占める椎間関節由来の疼痛の頻度は，約15％と報告されている¹⁾．山口県で行った腰痛の原因についての臨床研究でも，椎間関節は診断可能な，いわゆる非特異的腰痛の37％(非特異的腰痛全体の27％，腰痛全体の21％)を占めており，最も頻度の高い原因であった²⁾．現在，難治性の椎間関節性腰痛に対する後枝内側枝の高周波熱凝固(電気焼灼術)は“gold standard”の治療法として推奨されている³⁾．本稿では椎間関節性腰痛の診断，われわれが行っている複合筋活動電位(compound muscle action potential：以下，CMAP)を併用した電気焼灼術の実際について述べる．
