　リバース型人工肩関節(RSA)の適応とされている病態について，RSAに代わり得る治療法を検討した．偽性麻痺を伴う腱板断裂関節症はRSAの最もよい適応であるが，これに代わり得る術式として，小径人工骨頭置換術と肩甲下筋腱移行術の組み合わせがある．偽性麻痺を伴う広範囲腱板断裂においてRSAに代わり得る術式は，自家大腿筋膜を用いる上方関節包再建術である．関節リウマチでは，腱板が不良であっても解剖学的人工肩関節(TSA)を考慮する余地がある．高齢者の3パート，4パート骨折新鮮例に対して一次的にRSAが行われる傾向があるが，重要なポイントを押さえれば人工骨頭置換術で良好な成績が得られる可能性がある．
