誌上シンポジウム リバース型人工肩関節手術でわかったこと

本邦における術後早期合併症

　リバース型人工肩関節(RSA)が2014年4月から本邦で使用可能となった．その術中・術後早期の合併症について調査した．対象は2014年4月〜7月にRSAを施行された65肩である．平均年齢は76.1歳，平均経過観察期間は6カ月であった．病態は腱板断裂性関節症28肩，腱板広範囲断裂16肩，骨折後後遺症6肩，変形性肩関節症が5肩，新鮮骨折が5肩，関節リウマチが3肩，人工骨頭置換術後再置換術が2肩であった．術中合併症として，関節窩の骨折を1肩に認めた．術後早期合併症は，肩峰骨折，肩甲棘骨折，下方不安定症，感染をそれぞれ1肩に認め，発生率は6.2％であった．
