大腿骨頭壊死症に対する保存的治療の現状と展望

　大腿骨頭壊死症の最も重要な治療目標は，骨頭圧潰の発生・進行防止を通じて疼痛や機能障害低下を制御することである．免荷療法，運動療法は過去の臨床研究が乏しく，圧潰進行や人工関節置換などの手術治療を回避する効果は不明である．体外衝撃波治療や高気圧酸素治療は，少数の報告例であるが除痛効果を中心とした有効性が示されている．薬物療法では骨粗鬆症治療時と同等量のアレンドロネート療法に関する短期臨床効果の検証が整いつつあるが，疼痛軽減や圧潰発生/進行への有効性については報告間でばらつきがある．今後，より多くの症例を用い年齢・病態関連因子(ステロイド・アルコール・凝固線溶系機能異常など)や病期・病型に応じた解析を進めることが必要であるとともに，壊死領域に選択的に高密度に骨リモデリング作用を発現・制御し得るような治療手法の開発も重要な課題である．
