人工股関節置換術におけるショートテーパーウェッジ型ステムの固定様式の評価—ZedHipを用いた3次元的解析

背景：人工股関節置換術(THA)において，ショートテーパーウェッジ(STW)型ステムを用いる場合は近位での十分な髄腔適合が必要である．

目的：3次元画像解析ソフトウェアを用いてSTW型ステムの固定様式を評価，解析すること．

対象と方法：2013年8月以降，初回THAでSTW型ステムであるBiomet社製Microplastyステムを使用した21例22関節を対象とした．Gruenのzone分類における皮質骨とインプラントの接触の有無から固定様式を4群に分類し，各固定様式の各zoneで皮質骨と接したインプラント面積の割合を解析した．

結果：本例において90％でproximal-metaphyseal fittingが達成されていた．

まとめ：固定様式を正確に評価するうえで，3次元的な検討が重要になってくる可能性がある．
