仙腸関節障害の診断と治療

はじめに

　医師になって15年ほどは仙腸関節由来の痛みを知らずに，腰痛の専門医として何とかやってきた筆者には，“本当に仙腸関節の痛みはあるのか”，との疑問を持つ医療者の気持ちがよく理解できる．一方，この痛みの存在に気付いてからは，決して少なくないことに驚いている．仙腸関節由来は身体的特徴を把握すれば，みつけるのは決して困難ではないが，特異な画像所見が得られないために，見逃されやすい面を持つ．なかには身体表現性疼痛障害と診断される患者さんもいる．このような悲劇を生まないためにも，仙腸関節の痛みの特徴を知り，鑑別診断の1つに入れることが重要と考える．その理解の一助になればと思い，この疾患の特徴と診断および治療について述べたいと思う．
