腫瘍用人工関節感染に対して再置換術を行った6例の治療経験

背景：腫瘍用人工関節置換術は，術後感染率が高く，その治療には再置換術を要することが多い．

対象と方法：腫瘍用人工関節感染に対して再置換術を行った6例の治療法を検討した．起炎菌は全例メチシリン耐性黄色ブドウ球菌(MRSA)を認め，一期的，二期的再置換がそれぞれ半数ずつ行われた．

結果：最終的に5例で感染が鎮静化された．

まとめ：感染の波及した不良組織を積極的に除去し，血流のよい軟部組織再建を併用すること，および，ブドウ球菌感染に対してはリファンピシン内服を併用することで，年単位の排膿が持続していた症例も感染を鎮静化することができた．
