認知症患者の痛みの管理

　高齢者の多くが何らかの痛みを訴えていると言われ，介護施設入所者ではさらに高い．とくに認知症を有する高齢者のケアにあたり自立生活障害度が高い人に痛みの訴えの頻度がより高く，対応が難しい問題となっている．一方，認知症を持つ高齢者の疼痛の把握と評価が難しいという問題もある．把握・評価が難しいことに加えて脳病理変化により認知症高齢者は痛み知覚に対して鈍感になっているようにみえるとの意見もある．ここでは高齢者とくに認知症を持つ高齢者の痛みの訴えに関して，認知症による訴えの変化，認知症脳病理が痛み知覚に与える影響，認知症高齢者の痛みの医学的管理について述べた．合理的な薬物療法を進める際のAmerican Society for Pain Management NursingおよびAmerican Geriatric Societyにより提唱されている鎮痛薬などの使用ガイドラインを紹介した．また，痛みへの対処が認知症の問題行動(BPSD)を軽減する可能性についても言及した．
