—ガイドラインを考慮した治療—慢性腰痛

はじめに

　2012年10月に刊行されたわが国の腰痛診療ガイドライン(以下，ガイドライン)には，“Clinical Question 11：腰痛に薬物療法は有効か”という項目があり，これに対する回答として“Grade A：薬物療法は有用である”と明確な記載がある¹⁾．腰痛に対する具体的な治療薬としてガイドラインには，1．非ステロイド性抗炎症薬(NSAIDs)，2．COX-2選択的阻害薬，3．アセトアミノフェン，4．筋弛緩薬，5．抗不安薬，6．抗うつ薬，7．オピオイド，8．その他，が記載されている．それぞれの薬物にエビデンスが示されているが，これはCochraneレビュー，ヨーロピアンガイドライン，米国内科学会と米国疼痛学会によるガイドラインを基に，2008年3月31日までの期間に発刊された文献を検索した結果である．しかし初版のガイドラインの作成に当たり参考とされた約10年前の知見と現在とを比較すると，腰痛診療に対して使用される薬物はここ数年で大きく様変わりした感がある²⁾．本稿では，ガイドラインを踏まえたわが国の腰痛に対する薬物療法の現状，ガイドライン発刊以降の最新の知見を解説しつつ，慢性腰痛の診療に対する現状と今後の課題について筆者の考えを述べたい．
