—パーキンソン病と腰痛—パーキンソン病に対する高周波熱凝固術（RF-T）を中心とした複合治療—難治性腰背部痛を呈する神経筋疾患に対する腰髄神経後枝内側枝RF-Tの有効性

　パーキンソン病に代表される神経筋疾患(neuromuscular disorders：NMD)の腰背部痛(low back pain：LBP)は難治性であり，腰椎固定手術後に高い合併症が出現することが知られている．後側弯を来したNMD患者のLBPに対し，当院では除痛効果を目的とした腰髄神経後枝内側枝の高周波熱凝固術(radiofrequency thermocoagulation [RF-T] of the posteriomedian branch of lumbar spine：PBLS)を施行しているが，この手技のみでは長期間の除痛を得ることが困難な症例が散見された．現在は，RF-T of PBLSに加え，仙腸関節の高周波熱凝固術，梨状筋ブロックおよびボツリヌス毒素の筋肉注射などを組み合わせることで，RF-T of PBLSの単独治療と比較し長期的な除痛効果を認めている．いずれの手法でも奏功しない症例には最終手法として脊髄刺激療法を行い，除痛の相乗効果を得ている．NMDのLBPの病態は個々の症例で異なっており複数の病態が合併しているため，それぞれに適した複合治療が必要である．
