転移性脊椎腫瘍に対する新たな低侵襲手術　経椎弓根的腫瘍内切除による神経除圧の有用性

背景：転移性脊椎腫瘍が脊椎不安定性および神経根症状を呈すると，疼痛の制御は困難となる．

方法と対象：椎体腫瘍が神経根を圧迫し，腰痛および神経根症状で歩行困難を呈した2例に対し，椎弓切除と経椎弓根的に脊柱管内に膨隆した腫瘍の腫瘍内切除，減圧を行った．

結果：術中の腫瘍からの出血は容易に制御可能で，肉眼的に神経の除圧が確認可能だった．術後，神経根症状は改善し歩行可能となり，術後約3カ月の間，症状の再発は認めなかった．

まとめ：経椎弓根的腫瘍内切除により，低侵襲で腫瘍の切除および神経の除圧が可能となる．
