椎間板変性治療:多血小板血漿(Platelet-Rich Plasma:PRP)

 椎間板変性は炎症性サイトカインの異常発現を基盤とした組織変性が原因と考えられており,組織断裂に伴う疼痛や変性疾患の発症に関連する.多血小板血漿(PRP)は血小板に含まれる多種のサイトカインや成長因子を高濃度に含有しており,組織再生に臨床応用されている.我々は活性化PRPから得られた上清(PRP上清)が椎間板細胞を活性化し,細胞増殖および基質合成が促進することを報告した.基礎研究結果をもとに椎間板性疼痛患者に対する自己PRP上清注入治療の臨床試験を行い,PRP治療の安全性および予備的な疼痛の改善効果に関して報告する.
