ステロイド関連大腿骨頭壊死症の発生予防は可能か

はじめに

　特発性大腿骨頭壊死症(osteonecrosis of femoral head：ONFH)は整形外科領域の代表的な疾患であり，人工股関節や骨切り術といった手術治療がメインとなる整形外科らしい疾患である．ONFHの基本病態は虚血に伴う骨梗塞であり，特徴的なのは，骨頭に梗塞が“発生”してから，骨頭が圧潰して“発症”するまでに数カ月から数年のタイムラグが存在することである．ONFH患者が整形外科外来を受診する動機のほとんどは「股関節が痛い」からであり，初診時にはすでに骨頭が圧潰していることが多い．すなわち，日常臨床で整形外科医が治療対象としているのは“発症”しているONFHである．ONFHに対する“発生の予防”と“発症の予防”はともに重要な課題であるが，いまだ有効な方法は確立されていない．本稿では，“発生の予防”にフォーカスを当て，基礎実験から現在実施されている臨床研究までを概説する．
