骨粗鬆症脊椎骨折後偽関節に対する固定術後のX線学的経過不良例のリスク因子

　骨粗鬆症椎体骨折後の偽関節に関する手術成績の報告は，多くが術式間で比較した結果である．そこでわれわれは多施設研究を行い，本症固定術後のX線学的経過不良例を解析した結果，罹患高位が中下位腰椎であることが最も有意なリスク因子であった．これは解剖学的な理由などから十分なアンカー数の確保や前方支柱再建が困難であったことが要因と考えられた．近年，本邦に導入された低侵襲側方アプローチによる椎体置換を用いるなどして，高齢者であっても骨折の特徴に合致した適切なプランニングがこれら経過不良の予防には肝要である．
