第5腰椎分離すべり症に対するTransdiscal Screw Fixation Method

　近年，第5腰椎分離すべり症に対して，椎間スペーサーを用いた椎体間固定術を選択することが多い．しかし，骨質の脆弱な症例では，沈下現象から派生する偽関節や矯正損失が，しばしば問題となる．今回われわれは，椎間高が消失し，かつ矢状面アライメントが良好に保たれていた骨質不良の3例にtransdiscal screw fixationを施行したところ，矯正損失なく全例で良好な骨癒合が得られた．本法は通常の椎弓根スクリューに比べて，1.6〜1.8倍の固定性を獲得できるため，症例を選べば，骨質不良症例の治療の選択肢の1つになり得ると考える．
