リウマチ足のMTP関節温存手術は患者のADLをどのように変えたか

はじめに

　関節リウマチ(RA)の前足部では外反母趾，槌趾，開張足を合併し，進行すると中足趾節間(MTP)関節が背側に脱臼する扁平三角状変形を呈する．足底や趾の背側に有痛性胼胝(タコ)が形成されるため，履物障害や歩行時痛に悩まされる．筆者らはこのような足の再建に，切除関節形成術ではなく，1982年から始めた中足骨の頚部斜め骨切り短縮術Shortening oblique osteotomy(以下，SOO)¹⁾を行ってきた．新潟県立瀬波病院での中長期成績は1997年，Clinical Orthopaedics and Related Research誌に“Arthroplasty for rheumatoid forefoot deformities by a shortening oblique osteotomy”というタイトルで掲載された²⁾．また，母趾にflexible hinge toe(Swanson implant)を挿入し，外側4趾にSOOを施行したRA症例の長期成績(平均12年)は2001年にJournal of Orthopaedic Science誌に報告した³⁾．また，2011年，手術手技の改良点や成績を本誌に寄稿した⁴⁾．これらの成績は医師側からみた客観的評価である．

　そこで今回，長岡赤十字病院での手術症例で患者立脚型の足部足関節評価質問票(SAFE-Q)^5-7)によるアンケート調査を実施し，前足部変形に対する再建術の評価を行ったので報告する．
