慢性疼痛治療に対する脊髄刺激法

はじめに

　脊髄刺激法(spinal cord stimulation：SCS)は，1971年に初めて疼痛疾患に対して臨床使用されて以来，そのデバイスは改良を重ね，現在では，薬物治療抵抗性の難治性慢性痛の治療法として選択される機会が増加している．さらに，近年では，超高頻度刺激(10kHz)やバースト刺激などの新しい概念も導入され，低頻度刺激に特有の刺激感(パレステジア)を感じることなく，痛みを緩和することが可能になった．これまで推測されていた鎮痛メカニズム以外にも，いまだ解明されていない鎮痛メカニズムの存在も示唆されている．今後，SCSを用いた慢性疼痛治療は，新たな局面を迎える．
